UCSD is the only residency program in UCSD run by residents. At UCSD, YOU, not the fellow or the attending, protocol exams, read studies, and field consults and questions from clinical services. Thus, when you graduate from UCSD, you can run any service. You will be excellently trained to succeed in either an academic or private practice.
